When It's the Panel and When It's Not
It is honestly not always the panel, and we will tell you that straight. A single double-tapped breaker can be a small fix, adding a breaker, using a listed tandem, or pigtailing the wires, and a missing circuit label is a quick relabel. No main disconnect is not automatically a violation either, because older homes were legal under the historical six-disconnect rule and there is no retroactive enforcement on a compliant old install.
Where it does become a real replacement is when the panel is a Federal Pacific or Zinsco, when it is corroded or water-stained, or when those double-taps are just a symptom that an undersized service is full. And the moment service equipment is replaced, the panel has to meet today's California code, which usually means adding a proper main and the now-required outdoor emergency disconnect. So a marginal panel with several small write-ups is almost always cheaper and cleaner to replace once than to patch piece by piece.
